Exemplary Direct Patient Care Award

This award recognizes an individual who provides exceptional direct patient care serving as an inspirational role model for others.

winner: Stephen Rodgers, np

Stephen Rodgers, NP Award Winner

Stephen (left) has provided exemplary person-centered care at multiple CFH health care delivery sites. These include Susan’s Place, the Living Room, the Jerome Avenue Men’s Shelter, and the Franklin Women’s Assessment Center. He provides exceptional patient follow-up and his patients always request him by name. In addition, his deep bench of clinical experience in HIV treatment has been in crucial for many CFH patients receiving HIV specialty care throughout the city.

This award recognizes the outstanding contributions of a staff member who advances the practice of public health at CFH to help improve the well-being of the communities we serve.

winner: Isabel Odean

Isabel’s expertise in Public Health has been instrumental in the execution of many government and privately funded grant programs. Her work has also has enabled CFH to move forward with many innovative programs. These include open notes, PEP, PrEP, and Ending the Epidemic as well as COVID testing. During the height of the COVID-19 pandemic period, she worked with Population Health to create an internal call center program, to effectively implement telemedicine, assuring continued patient access.

This award recognizes an individual that works tirelessly to educate our clients about the importance of receiving vaccinations.

winner: Steven Tapley, NM

Steven’s role as the vaccine coordinator increased exponentially this year with the availability of COVID-19 immunizations. Since the pandemic began, he has led and scheduled the COVID-19 events in which both the Moderna and J&J vaccines were distributed. This level of work is comparable to that of an entire team’s and Steven consistently performs these responsibilities admirably. And as a result, vaccinations at Care Found Here – Junius Street have been through the roof. Congratulations on all you have accomplished, Steven!

This award recognizes an individual who understands the mission of CFH and authentically demonstrates leadership in putting those principles into practice.

winner: Romona Watson

Romona is a wonderful colleague, contributing to CFH immensely on a daily basis. She goes above and beyond to provide assistance to everyone at the health centers. Additionally, her innovative approach to improve our Health Information Management capabilities has dramatically improved workflow and taken CFH to the next level.

winner: Genesis Reyes

CFH Peer Outreach Specialists

Genesis (center) is extremely compassionate towards the patients that stop by our health centers. She is always a pleasure to work with because her positive attitude can make any responsibility fun and interesting. Genesis possesses a natural talent for working with our patients. She goes above and beyond for them, and they sincerely trust her.

Trauma-Informed Care Champion Award

This award recognizes an individual who has contributed to the advancement of trauma informed care principles at CFH to create a safe, reliable, patient-centered environment for our clients.

winner: Judith Coffey, NP

Judith significantly and consistently contributes to CFH’s organization wide, Trauma Informed Care (TIC) Initiative. She has provided essential internal leadership in ongoing staff engagement and education on this topic, enhancing our staffโ€™s ability to understand the trauma many of their patients experience.

HIV/AIDS Education Champion Award

This award recognizes a staff member who does patient outreach and engagement with the goal of educating and empowering our clients to understand the importance of HIV detection and prevention.

winners: Ilkia Solano & Michael Falcon

Ilkia Solano

Ilkia (right) is an extraordinary public health educator, especially when it comes to HIV detection and prevention. She is also exceptionally skilled in HIV measurements and care follow up. Ilkia is able to connect with patients and gain their trust, encouraging them to receive vital health services. She brings positive energy into everything she does, especially population health events.

Michael (right) has done a wonderful job working with the Population Health Management Team in a fun, efficient, and effective way. He is a strong leader with exceptional communication and organization skills. He is also the go to HIV contact for all of CFH.

Winners are recognized by their peers as outstanding assets to the organization, who have demonstrated significant contributions to the work environment of the health care team.

winner: Sharon Darlington

Sharon has been working at the 30th St. Menโ€™s Shelter since the CFH health center opened and has been an incredible member of the team ever since.  She is kind, caring, and hard-working. One can always go to Sharon with a question, and she is always willing to help you figure it out. Sharon has also been the stabilizing force for the 30th Street practice throughout the COVID-19 pandemic. Her quiet perseverance has been critical to the continued delivery of essential services for an exceptionally vulnerable population.

Nora Lueth CFH Award Winner

Nora has been instrumental in building the Care Found Here – Junius Street practice. She provides a grounding presence for staff and has played an important role in the implementation of telemedicine coverage at multiple CFH sites. She has a positive and creative approach to problem solving, and an ability to create internal patient support teamsm significantly improving outcomes for CFH patients.

winner: Lenda Estrella

Lenda inspires the entire health center staff every week with several inspirational messages. They’re a CFH mainstay! She has also provided essential coverage for multiple sites and consistent support for the POTS site, specifically during a long shortage period among the nursing staff. We thank her for her positive attitude and team spirit. Lenda always offers help, acts as a constant ear for her peers, and offers advice. Put simply, she is the backbone of the CFH Bronx support staff!

S/HERO of the Year Award

This award recognizes an individual that went above and beyond this year, to provide care to our clients and support fellow colleagues, serving as an inspiration to all at CFH.

winner: Judson York, Nurse manager

Judson has been providing outstanding support throughout the past year, at all 26 CFH health care delivery sites. He ensures they stay supplied, facilitates the openings of new sites, leads immunization initiatives, orients new staff, and more! Thank you, Judson, for your commitment to the CFH staff, patients, and shelter residents. Judson holds the Bronx (and other health centers!) together with his adaptability and heโ€™s always available to answer a phone call (whenever, wherever) to support his staff. We’d be lost without him. He truly is the hero of the year!
