We provide high-quality, patient centered health care to all those experiencing homelessness regardless of their ability to pay.

We encourage all our patients to talk to us about their health history, any health care needs and goals, as well as any other questions or concerns. We promise to listen, provide clear explanations about your care, help you set goals to improve your health, and provide interpretation services for any language or physical impairment.

  • Services are available for women, men, and children.
  • You will be treated regardless of your ability to pay. A sliding scale fee schedule is available based on family size and income.
  • We will help you apply for health insurance.
  • You have a personal care team working to improve your health.
  • You choose your primary care provider (PCP).
  • Your PCP coordinates all your health care needs, including behavioral health and substance abuse counseling.
  • Your PCP can refer you to see specialists and follow-up with them.
  • Your medical records are available to you in all CFH health centers and online via our Patient Portal.


Ask about our Patient Portal and Healow App for free, easy, and private access to your personal medical and medication information. All you need is an email address.



Our primary care services provide patients with physicals, vaccinations, and health screenings. We additionally provide services that help manage chronic diseases, a cold or flu, and non-emergency issues.
