Primary Care Services

Our primary care services provide both adult patients with physicals, vaccinations, and health screenings. We also provide chronic disease management care as well as cold, flu, and non-emergency issues on-site medical care.

In addition, our pediatric servicesย include health and wellness care, nutrition counseling, age appropriate immunizations, and episodic illness care.

CFH also offers primary care management and care coordination for patients who identify as LGBTQ as well as those with HIV.
