5 CFH Sites Receive PCMH Designation

We are excited to announce that the NCQA has designated five additional CFH health centers as Patient-Centered Medical Homes (PCMH).

We are excited to announce that the National Committee on Quality Assurance (NCQA) has designated five additional CFH health centers as Patient-Centered Medical Homes (PCMH). 

The five centers are 30th Street (Manhattan), 52nd Street (Manhattan), Laurel Hall (Queens), and Junius Street (Brooklyn), and the Living Room (the Bronx). This brings CFH to a total of 18 health centers with PCMH designation. This is in addition to 13 of our health centers receiving the PCHM designation for behavioral health.

What is a PCMH Designation?

The PCMH designation indicates that these health centers make patients their top priority and promote healthy relationships between patients and providers. In addition, research has shown PCMHs improve health care outcomes, increase staff satisfaction, and keep health care costs low.

We believe that people experiencing homelessness deserve the same quality health care as everyone else, or better. And, if youโ€™re familiar with us, you know we always say we provide โ€œhigh quality health care to New Yorkโ€™s neediest residents.โ€ Of course, we stand by that statement. What is particularly notable about this news? The PCMH designation is an objective, third-party assessment of the quality of care provided by our health services teams.

This designation shows that we โ€œwalk the talkโ€.
