The Care for the Homeless Social Service Team provides direct services to clients to help them stabilize and maintain the gains that they make for themselves, their families and their communities and to bring their present episode of homeless to a close.
To do this, the Social Service Team provides both Intensive and Supportive Case Management Services to the neediest homeless clients in NYC. Team members conduct outreach and work in those places where homeless people congregate in numerous sites across four boroughs including drop-in centers, single/family shelters, soup kitchens and medical clinics.
By offering psychotherapy for clients, pharmacological and non-pharmacological interventions as well as after-care to ensure the client's success, CFH works to provide for the unique needs of each client.
Mental illness is a great contributor to homelessness, and mental health services attempt to break the vicious cycle by educating and treating homeless men, women and children.
