The Executive Assistant will perform a variety of secretarial duties

ESSENTIAL DUTIES AND RESPONSIBILITIES

Managers.
• Compose basic correspondence when required.
• Prepare and maintain excel spreadsheets as requested.
• Answer phones, screens calls, provide information and direct
calls to proper parties.

• May develop and maintain computerized spreadsheets and databases

JOB QUALIFICATIONS
Educational Requirements:
• High School Diploma.
Experience:
• 2-3 years secretarial/administrative experience working
with underserved populations.

CLINICAL SUPERVISOR
The Clinical Supervisor will provide leadership, supervisory and
direct case management services while engaging homeless women with
appropriate concern for their social, mental health, substance abuse
and medical problems in order to promote client stability and empower
them to reach their fullest potential.
ESSENTIAL DUTIES AND RESPONSIBILITIES
CASE MANAGEMENT DUTIES
• Directly maintain a small caseload of seriously mentally
ill and/or medically frail clients which are highly resistant to
assist to stabilize and promote housing goals.
• Review logbooks at start of each shift to ensure that all
clients follow up is conducted as required.
• Participate as part of a team and regularly collaborate
with multidisciplinary staff to monitor all clients’ progress.
• Collaborate with internal staff to monitor clients’
medication compliance and address episodes of non-adherence.
• Provide education, advocacy and direct assistance for individual
clients around entitlements and services.
• Provide ongoing case management, including individual counseling,
crisis intervention, conflict resolution, advocacy, psychoeducation,
coordination of services and appropriate follow-up on all referrals
to all clients.
• Use motivational interviewing and seeking safety modalities
to counsel clients to improve overall functioning.
• Encourage and provide ongoing support so that each client
might assume an active role in their own treatment plan to the maximum
extent possible.
• Maintain required documentation in case records in written
and computer database (SCIMS, AWARDS) and update clients records
in accordance with governmental and agency policy.
• Assist in completion of Housing applications (both written
and computer generated) for clients based on their individual needs
and functioning.
• Facilitate and assist with clients' transfers into other
transitional shelters or independent living.
• Participate in weekly case conferences with internal/external
departments providers to assess clients’ progress.
• Participate in regular supervision, team meetings, case
conferences and staff trainings.
• Maintain a flexible schedule which will include evening
and weekend shifts.
SUPERVISORY DUTIES
• Directly supervise 3-5 case managers and/or interns.
• Provide orientation and initial training to new social service
staff recruited.
• Provide ongoing training and individual supervision to social
service staff.
• Oversee the accurate completion of client intakes and ensure
that orientation to the program and shelter rules and regulations
was provided for all new clients by social service staff.
• Review and cosign comprehensive psychosocial assessments
developed by social service staff to ensure they are accurate and
fully identify and address the client's needs.
• Review and cosign individualized intervention/treatment
plan developed by social service staff to ensure accurate completion
and promote identified goals.
• Oversee the facilitation of clinical, educational, supportive
and/or recreation groups conducted by social service staff to ensure
that groups held address problems areas that clients experience
in working towards independent living.
• Audit social service staff group notes for accuracy, content
and to ensure deadlines are met as indicated.
• Complete supervisory notes, performance evaluations for
staff and develop goals that promote professional development.
• Provide clinical and supervisory expertise to case management
and shelter staff.
• Monitor supervisees’ completion of departmental requirements
including housing applications, face to face sessions, housing placements,
etc...
• Conduct rounds throughout the facility regularly during
shift, to assess clients’ interactions, address any client
conflicts and attend to clinical needs.
• Provide crisis intervention and utilize non-violent conflict
resolution techniques to deter conflicts among clients.
• Facilitate case conferences with supervisees to address
critical clinical issues and non-adherence to facility rules and
regulations.
• Conduct regular chart audits to support quality assurance
practices.
• Work in collaboration with Director of Social Services to
assess program progress and develop new policies and procedures.
• Assess and oversee client matters that warrant emergency
response services and represent the agency in communicating to EMS,
NYPD, FDNY to effectively communicate client’s clinical needs
and issues of concern.
• Directly assist with internal fire drills to assure all
clients’ comply with this requirement and intervene/educate
any clients that are in violation of this procedure.
• Directly collaborate with onsite medical staff to address
all urgent/emergency medical and psychiatric client issues that
arise.
• Oversee the completion of the weekly updates for all clients
to funders.
• Directly assist with internal and external reports in compliance
with deadlines set.
• In absence of Director of Social Services, the Clinical
Supervisors will provide direct support to case management staff
and assume administrative and supervisory responsibility for all
departmental activities related to the operation and full functioning
of the department.

JOB QUALIFICATIONS

• Master’s Degree in Social Work, Psychology, Counseling
or other related field required.
Experience:
• Minimum 7-8 years post graduate social work experience with
a minimum of 5 years supervisory experience required. Experience
dealing with homeless, mentally ill and substance abuse populations
preferred.

The Medical Case Manager will provide direct case management services

are medically frail and require daily monitoring and ongoing assistance.
• Complete intakes for new clients and provide orientation
to the program and shelter rules and regulations.

client follow up is conducted as required.
• Participate as part of a team and regularly collaborate
with internal medical staff to monitor client's progress and ongoing
needs.

HEALTH EDUCATOR
The Health Educator will provide counseling and educational support
to clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES
• Implement HIV rapid testing program at assigned outreach
sites;
• Assess individual and shelter-wide health education and
HIV testing needs at assigned outreach sites
• Perform outreach to homeless clients and facilitate their
participation in on-site medical and social services, health education
interventions and HIV testing;
• Develop and maintain relationships with staff of contracted
medical and dental providers, and social service staff at sites,
and inform them about health education and HIV testing and programming;
• Provide HIV pre- and post-test counseling, referral, and
partner notification services to clients in compliance with all
applicable laws;
• Perform rapid HIV antibody tests, in compliance with all
applicable laws and regulations;
• Perform confirmatory HIV testing using oral fluid samples;
• Collect data to evaluate all health education services as
needed;
• Conduct approved group level interventions at assigned service
sites;
• Facilitate health education classes on topics that may include
sexual health, oral health, nutrition, and chronic disease;
• Create/order educational materials, rapid HIV testing supplies,
and other program supplies;
• Provide counseling to persons with serious health concerns,
including HIV and risk factors for HIV;
• Document all activities using encounter forms, program logs
and/or the AIRS reporting system;
• Submit all written documentation to supervisor for review;
• Educate clients, as needed, about their Medicaid and health
care rights;
• Act as a resource person in health education to clients
and staff;
• Stay abreast of new developments in health promotion and
disease prevention, particularly HIV risk reduction;
• Attend and participate in meetings, trainings and staff
development; and
• Perform other duties as assigned.
JOB QUALIFICATIONS
• Bachelor’s Degree preferred
• 2 years prior experience working in health care or social
services.
• Prior expertise/training in health education preferred.
• Strong desire to learn and enter this field if lacking experience
in this specific field.

OTHER REQUIREMENTS
While performing the duties of this position, the employee
travels regularly by automobile, subway or bus in four (4) New York
City boroughs and is exposed to changing weather conditions. The

Spring, Summer and Fall 2010 Policy and Advocacy interns
Interns will work directly with the Director of Policy.
They will be introduced to the wide range of issues currently affecting
the provision of health-related services, including but not limited
to Medicaid reform, mental health and substance abuse, health education,
HIV/AIDS, the health care delivery infrastructure, and shelter and
housing reforms. Internships, however, will focus on one issue,
determined by the interests and qualifications of the intern/s.
It is expected the intern/s will develop expertise in one area.
The Director of Policy also coordinates the activities of the NYC
Providers of Health Care for the Homeless, a coalition of the homeless
health centers which advocates for the health care rights of homeless
people in New York. Interns will be asked to complete a series of
tasks to develop the skills necessary for successful policy and
advocacy work and will either assist or themselves produce various
policy related materials during the internship.
Interns will be exposed to working with coalitions, collaborative
partnerships and alliances, and with policy decision-makers.
Interns should expect to assist with administrative functions
and should have excellent research and communication skills, be
well organized and self-directed.
