CHIEF FINANCIAL OFFICER


ESSENTIAL DUTIES AND RESPONSIBILITIES

Oversee Financial Management of Care for the Homeless
• Put all necessary systems in place to manage all the financial activities of CFH and its allied corporations maintaining compliance with the highest ethical standards, all applicable laws, regulations and requirements of funders
• Manage and supervise the Business Office and its personnel
• Update and maintain Financial Policies and Procedures
• Strengthen and maintain internal controls to manage cash flow
• Maximize revenue from Medicaid from billing of Medicaid, Medicare and Managed Care Organizations and non-Federal sources, including investments
• Put in procedures to control and reduce expenses, as applicable
• Oversee preparation of accurate and timely regular financial reports for review and decision- making with the Executive Director and Board of Directors’ Finance and Audit Committees
• Develop and manage the agency budget process
• Interact with outside auditors to conduct annual audit
• Train program management staff in fiscal concepts and skills to enable them to manage department and grant budgets;

Participate in Senior Management
• As member of Senior Management Team, help to develop and implement strategic plans for CFH
• Participate in planning to diversify funding streams for CFH
• Staff, along with the Executive Director, the Board of Directors’ Finance and Audit Committees

QUALIFICATIONS
• BA level with accounting courses and eight years progressive management experience with not-for-profit accounting.
• Knowledge of computer software, particularly MS Excel and MS Word
• Knowledge of MIP (not-for-profit accounting software) preferable.
• Graduate degree in accounting, business and/or CPA preferred.

DIRECTOR, SUSAN'S PLACE
CARE FOR THE HOMELESS operates Susan’s Place, a homeless residence and medical clinic, and provides medical and social services at thirty shelters, soup kitchens, and drop-in centers.

Responsibilities
• Responsible for the overall development and management of all program operations at the facility
• Serves as primary program liaison to DHS
• Works with Human Resources in fulfilling the staffing and personnel needs of the shelter i.e. employment, training, professional development according to the policies & procedures established at CFH
• Assures compliance with City and State regulations
• Directs the oversight of Pre-Program residence
• Assesses program needs and identifies potential funding streams to enhance services
• Serves as liaison to other service providers and community leaders to ensure the integration of the facility as a community-based agency
• Assures that the physical environment of the shelter is maintained and responds to quality assurance reports in a timely manner
• Responsible for contractual compliance with program goals and objectives
• Operates the facility and program within approved budget limit
• Direct supervision of the following personnel: Operations Director, Social Services Director and the Office Administrator
• Serves as part of the Senior Management of Care for the Homeless

Job Qualifications
• Minimum of 10 years in human services including administrative responsibilities

LICENSED CLINICAL SOCIAL WORKER
The Social Worker will provide leadership and direct case management services to homeless clients/families in order to promote client stability and empower them to reach their fullest potential. In absence of Director of Social Work and Clinical Coordinator, the Social Worker will assume administrative and supervisory responsibility for all departmental activities related to the operation and full functioning of the department.

ESSENTIAL DUTIES AND RESPONSIBILITIES
CASE MANAGEMENT DUTIES:
• Maintain a caseload of clients with co-morbid disorders.
• Develop a comprehensive psychosocial assessment to fully identify and address the client's needs.
• Provide individual counseling to clients that are referred to external providers for services or work with established providers and develop intervention/treatment plan or strategy to promote identified goals.
• Provide ongoing case management, including individual counseling, crisis intervention, conflict resolution, advocacy, treatment education, coordination of services, ensure that clients return for clinic follow-up appointments, and keep specialty care referrals.
• Use evidence based interventions such as cognitive behavioral, motivational interviewing, seeking safety to counsel clients.
• Encourage and provide ongoing support to clients on self management goals.
• Review clinical notes routinely and ensure that client follow up is conducted as required.
• Provide advocacy for clients and/or families around benefits/entitlements.
• Use a team approach through regular collaboration with multidisciplinary staff, facilitate clients’ health care needs are adequately met.
• Address episodes of non-adherence.
• Maintain required documentation in EMR database (eCW) and update clients records in accordance with governmental and agency policies.
• Facilitate and assist with clients' transitions into permanent housing including follow up on client’s progress and adherence to community treatment.
• Directly provide escorts to clients when necessary to scheduled appointments.
• Participate in regular team meetings with both internal departments and external providers.
• Participate in patient advocacy as necessary.
• Participate in regular supervision, team meetings, case conferences and staff trainings.

SUPERVISORY DUTIES:
• Directly supervise 2-4 case managers and/or interns.
• Provide orientation and initial training to new social service staff.
• Provide ongoing training and individual supervision to social service staff.
• Co-facilitate and oversee the facilitation of clinical, educational, supportive and/or recreation groups conducted by social service staff.
• Audit social service staff progress/ group notes for accuracy, content and to ensure deadlines are met as indicated.
• Complete supervisory notes, performance evaluations for staff and develop goals that promote professional development.
• Provide clinical and supervisory expertise to case management and site staff.
• Monitor supervisees’ completion of departmental requirements such as utilization and/or productivity, collaboration with site staff.
• Facilitate case conferences with supervisees to address clinical issues and provide guidance consistent with governmental, funder and agency policies and procedures.
• Conduct regular chart audits to support quality assurance protocols.
• Work in Collaboration with Director of Social Work to implement social services at new sites, conduct site visits regularly, address any site challenges and attend to site needs.
• Directly collaborate with site medical/behavioral health staff to address urgent/emergency medical and psychiatric problems.
• Directly assist with internal and external reports in compliance with established deadlines.
• Maintain a flexible schedule, to include evenings, weekends and Holidays.
• Perform other duties as directed.

JOB QUALIFICATIONS
Educational Requirements:
• Master’s Degree in Social Work, Psychology, Counseling or other related field required.
• Actively Licensed in state of New York as a Clinical Social Worker (LCSW)

Experience:
• Minimum 3-5 years post graduate social work experience with a minimum of 3 years supervisory experience required. Experience dealing with homeless, mentally ill and substance abuse populations preferred.

Skill Sets:
• Strong leadership, clinical and program implementation skills necessary.
• Strong relationship building skills required.
• Excellent organizational and time management skills necessary.
• Ability to effectively multi-task and follow-up on a timely basis.
• Proficiency with computers including Word and Excel required.

MEDICAL DIRECTOR
Reports to: Assistant Executive Director

Job Summary: The Medical Director is appointed by the Board of CFH; is delegated the authority and is responsible for the professional, organizational and administrative aspects of the adequacy and quality of health care provided to patients and clients of CFH. In particular, patients of CFH Article 28 clinics, CFH shelter facilities, and patients served in Section 330h grant funded areas.

Duties and Responsibilities:

Administrative/Supervisory Responsibilities

1) Establishes the standards of care and maintains the medical policies and procedures of the clinics consistent with NYS DOH Article 28, HRSA BPHC 330h and HIV/AIDS Bureau guidelines.
2) Directs compliance of medical services and quality improvement activities including Quality Review and Credentialing, patient Grievances and all medical activities in order to achieve a high quality of care and cost containment goals.
3) Ensures that resources are used efficiently, and that providers are used in ways to maximize their contribution to meeting patient and clinic needs.
4) Is a member of the CFH Senior Leadership and participates in determining the strategic direction of Care for the Homeless as it relates to medical services.
5) Actively participates in the hiring, orientation, evaluation and discipline processes for medical staff.
6) Insures ongoing medical services at all clinic sites.
7) Provides guidance and review of health related curricula.
8) Addresses health needs at non-Article 28 sites such as the shelter portion of Susan’s Place.
9) Establishes relationships/partnerships with community organizations such as other institutions of higher learning, social service agencies, tertiary institutions and other collaborators to enhance the health care services to CFH clients.
10) Participate in finding new funding sources and expanding current funding.

Clinical Responsibilities
1) Some percentage of time will be spent directly providing medical care and follow up interventions to clinic patients
2) Provides ongoing supervision for all providers in the CFH network and serve as collaborating physician for Nurse Practitioners as required by NYS Department of Education.
3) Lead or provide direction in clinical conferences, clinical incentives including, but not limited to, Meaningful Use, Patient Centered Medical Home; and provide medical expertise as appropriate.
4) Lead as directed or participate in the achievement of clinical standards as set by CFH.
5) Lead as directed or participate in meeting quality goals as set by CFH and funding sources including BPHC and Ryan White HIV funding.

Education and Experience
1) Current license as a physician in New York State.
2) Board certification / eligibility in a primary or specialty care discipline.
3) Refined organizational skills and leadership qualities
4) Experienced with health care planning and administration within an ambulatory health setting serving a multi-problem clientele.
5) FQHC experience.
6) Knowledge of medical staff organization and structure, medical credentialing and privileging, outcomes-based quality of care, clinical effectiveness and performance improvement.
7) EMR background.

Other:
This position will require traveling between clinic sites (up to several per day) and CFH central office, as needed. CFH has office / clinic locations in Brooklyn, the Bronx, Queens and Manhattan.

PSYCHOTHERAPIST - CASAC
The Psychotherapist (CASAC) provides direct psychotherapy, alcohol/substance abuse counseling/treatment and case management services to homeless clients, individuals and families that will address social, medical, mental health and substance abuse problems. Will assist clients in setting goals that will promote stability and empower them.

ESSENTIAL DUTIES AND RESPONSIBILITIES

CLINICAL DUTIES
• Maintain a caseload of clients who have co-morbid substance abuse issues and need therapeutic intervention.
• Develop a comprehensive psychosocial assessment to identify and address client’s needs.
• Complete a comprehensive substance abuse assessment of clients to identify needs and treatment goals.
• Provide direct psychotherapy to clients and establish an individualized intervention/treatment plan or strategy to promote identified goals.
• Provide psychotherapy and case management, including individual counseling, crisis intervention, conflict resolution, advocacy, psychoeducation, coordination of services and appropriate follow-up on all referrals.
• Engage clients in substance abuse counseling and treatment and other rehabilitative services.
• Use cognitive behavioral, motivational interviewing, stages of change, seeking safety and other client centered therapeutic interventions to counsel clients.
• Provide substance abuse counseling services to clients in sessions including education on harm reduction, relapse prevention, abstinence, etc…
• Encourage and promote self management and provide assistance to build self management skills.
• Review clinical notes routinely to ensure that clients follow up is conducted as required.
• Provide advocacy for individual clients and families around benefits and entitlements.
• Utilize team approach, through regular collaboration with multidisciplinary staff and facilitate clients’ health care needs are sufficiently met.
• Maintain required documentation in EMR database (eCW) and update clients records in accordance with governmental and agency policy.
• Facilitate clinical and/or substance abuse groups to address addictive behaviors and promote relapse prevention.
• Complete group progress notes as required by agency policy.
• Identify, network and work collaboratively with community substance abuse service providers and maintain an up-to-date directory of these resources.
• Initiate contact and maintain referral relationships with State, City and private agencies that provide substance abuse treatment services including residential modalities.
• Arrange transportation to intake interviews at treatment programs and treatment related appointments for clients.
• Directly escort clients to treatment interviews and/or treatment related appointments when necessary.
• Participate in regular team meetings with both internal departments and external providers.
• Participate in regular supervision, team meetings, case conferences and staff trainings.
• Maintain a flexible schedule, to include evenings, weekends and Holidays.
• Perform other duties as required.

JOB QUALIFICATIONS
Educational Requirements
• Master’s Degree in Social Work, Psychology required.
• Must possess a valid License in Clinical Social Work or License in Psychology for the state of New York
• Must have an active CASAC license for the state of New York

Experience
• A minimum of three (3) to five (5) years post graduate experience working with mentally ill, medically frail, homeless, substance abuse or special needs population required.
• Experience and familiarity with various substance abuse and clinical treatment modalities and resources required.

Skill Sets
• Must be flexible and able to work in non-traditional settings.
• Must be able to commute in all 5 boroughs of NYC as necessary.
